The "Wellness Assessment Questionnaire" is a diagnostic questionnaire that could very well be one of the most important tools available for you and for your patients!

Currently we have taken the 14 major systems of the body and broken them down into 81 different sub-categories and matched them against 50 of the most common symptoms or ailments presenting in our offices today!

This service also has a "General" category questionnaire that can be used on every patient. It consists of less than 200 questions and will evaluate and cross link all of the 81 major sub-category systems to detect what is happening in the patient. It will identify what is currently developing in your patients and make the specific Nutri-West product recommendations to help them correct and regain their optimal health!
